Send it to the people who know the child
Ask a caregiver, who answers in the portal. Send a teacher a private link, with no account to create. Or type the answers yourself from an interview.

The Questions About Behavioral Function asks the people who know the child how often a behavior happens in 25 situations. Nūr Evoke sends it, scores the five functions out of 15 and draws the graph, whether a caregiver answers in the portal, a teacher answers through a link, or you enter the answers yourself.
Every completed form is graphed and kept, so a repeat QABF sits beside the last one. These are samples with made-up data.
Everything between choosing the behavior and having the graph for your report.

Each of the 25 statements is rated never, rarely, some or often, or marked as not applying. Answers are saved as the person goes, so they can finish later, and every statement must be answered before it can be sent back.
Choose one of the client’s behavior-reduction programs, or type a behavior name. To ask about three behaviors, send three forms.
The five functions (attention, escape, non-social, physical and tangible) each get five statements and a score out of 15. Beside each score is how many statements were answered, and a statement marked as not applying is never turned into a number.
A bar graph of the five scores, with the highest marked. Ties are all marked, and nothing is marked when every score is 0. Save the graph as an image or print it.
Every completed QABF is its own dated set on the graph, so you can compare raters or see how answers change over time.
Print or download a completed QABF as a PDF for the client’s record.
Emails to caregivers and teachers say only that a behavior questionnaire is waiting, never the client’s details, and reminders continue until it is done.
Open the client’s QABF tab, pick the behavior, and choose who should answer: caregivers, a teacher by link, or yourself.
Each person rates the 25 statements on their own device. A BCBA can also enter answers from an interview.
Nūr Evoke adds up the five functions and shows how many statements were answered for each.
The BCBA reads the graph, saves it as an image or prints it, and keeps the PDF of the completed form.
The Questions About Behavioral Function is an indirect assessment of why a behavior happens. A rater scores 25 statements, five for each of five functions: attention, escape, non-social, physical and tangible.
Yes. Send the teacher a private link. They answer on their own device, and the answers go to the client’s QABF tab.
No. It adds up the scores, draws the graph and marks the highest. Interpreting the result is your BCBA’s job.
Yes. Each completed form is its own dated set, so you can compare raters or repeat it later and see the change.
Admins and BCBAs (for their assigned clients) send the QABF, enter answers and read results. RBTs assigned to the client can view the results. Every view and change is written to the audit log.
No. Questionnaire answers are kept apart from session data, so they never change program graphs, mastery or reports.
