Send it to the people who know the child
A caregiver answers in the portal, a teacher answers through a private link with no account, or you enter the answers yourself from an interview.

The Functional Analysis Screening Tool asks yes-or-no questions about a behavior. Nūr Evoke sends it to caregivers and teachers, asks only the parts that apply, counts the Yes answers in each of the five scoring rows and draws the chart, with one bar for every informant.
One bar per informant, or the average when several people have answered. These are samples with made-up data.
From choosing the behavior to the chart you paste into your report.

Each informant says how they know the person, for how long, whether they see them daily and in which situations, so the chart shows who each bar came from.
Choose the behavior type. For self-injury and repetitive behavior, the person answers Part I, then Part II only if Part I calls for it, then Part III. For aggression and other socially disruptive behavior, they answer Part II only.
Questions that ask for it take a short note, such as which item or activity, and there is a comments box at the end. Answers are saved as the person goes.
Nūr Evoke counts the Yes answers for attention, access to items or activities, escape, sensory stimulation and pain attenuation, as the form’s scoring summary does. Rows that were not asked say so and are never shown as 0.
A column chart of the number of Yes answers in each row, with one bar per informant in black and gray. With several informants, switch to the average. Save the chart as an image or print it.
Every completed FAST is its own dated set, so you can add informants or run it again later.
Print or download a completed FAST as a PDF for the client’s record.
Open the client’s FAST tab, pick the behavior and its type, and choose who should answer.
Each person tells you how they know the client, then answers the questions that apply to the behavior type. A BCBA can also enter answers from an interview.
Nūr Evoke counts the Yes answers in each of the five rows.
The BCBA reads the chart, compares informants or looks at their average, and saves it as an image or prints it.
The Functional Analysis Screening Tool is a short, yes-or-no interview about a behavior. The Yes answers are counted in five rows: attention, access to items or activities, escape, sensory stimulation and pain attenuation.
Yes. Each informant gets their own bar in the chart, and with several informants you can show the average instead.
No. It counts the answers, draws the chart and marks the highest rows. Interpreting the result is your BCBA’s job.
Yes. Send the teacher a private link. They answer on their own device, and the answers go to the client’s FAST tab.
Admins and BCBAs (for their assigned clients) send the FAST, enter answers and read results. RBTs assigned to the client can view the results. Every view and change is written to the audit log.
